RE9 has 2 puzzle boxes — one in the Chairman's Office (Care Center) and one in the Lead Researcher's Office (ARK Facility). Both reward weapon attachments and are required for trophy completion. Step-by-step symbol combinations below.

How RE9 puzzle boxes work

Each puzzle box has 4 faces with carved symbols. Rotate the box (left-stick on controller, mouse-drag on PC) to align the correct symbol to the gold-rimmed receptacle on each side. When all 4 sides match correctly, the top lid pops open.

The symbol set is the same across both boxes (8 symbols total: Sun, Moon, Eye, Skull, Snake, Tree, Heart, Star), but the correct combination is different per box and per playthrough difficulty.

Puzzle Box #1 — Chairman's Office (Care Center)

Located on the chairman's desk in the East Wing, second floor. Accessible after Chunk #1 boss fight. Reward: SMG Suppressor attachment.

💡 Hint location: The chairman's desk drawer contains a journal page showing the symbol order. Read it before solving — saves trial-and-error.

Puzzle Box #2 — Lead Researcher's Office (ARK Facility)

Located in the ARK Facility's lab levels, on the lead researcher's bookshelf. Accessible after The Commander boss fight. Reward: Shotgun Choke attachment (tightens MSBG 500 spread, +30% effective range).

💡 Hint location: The lead researcher's desk has a notebook taped under the second drawer. Pull the drawer fully out to find it.

Puzzle box solving tips

  1. Don't brute-force. The symbol combinations are puzzle-specific — your Care Center solution won't work in the ARK box.
  2. Find the hint papers first. Both puzzle boxes have hint documents in the same room. Look for them before touching the box.
  3. Rotation is intuitive. Left-stick (controller) or mouse-drag (PC) rotates one face at a time. The box stays centered.
  4. Wrong combinations don't punish you — no damage, no time penalty. Just keep trying.
  5. The Insanity solutions are different. If you're replaying on Insanity, the codes above are confirmed for v1.02.

Missable warning

⚠ Both puzzle boxes are 100% missable. Once you leave the Care Center for the Courtyard, Puzzle Box #1 is permanently locked for that playthrough. Same for Puzzle Box #2 once you trigger the Victor Gideon cutscene.
